Heal at Home provides Indiana Medicaid‑covered home health services for eligible adults, helping individuals receive medically necessary care in the comfort and safety of their own homes. Services are delivered under a physician’s order and follow an individualized plan of care focused on recovery, stability, and maintaining independence.

 

We serve adults enrolled in Indiana Medicaid programs, including PathWays for Aging and Hoosier Care Connect, and work closely with physicians, care managers, and managed care entities to ensure services are coordinated, compliant, and person‑centered. Our experienced care teams support adults managing chronic conditions, recovering from illness or hospitalization, or needing skilled oversight to remain safely at home.

 

Heal at Home also supports eligible individuals and families through Medicaid waiver program services, including respite services, when authorized as part of an approved care plan.

 

We assist patients and caregivers in understanding eligibility, service authorization, and next steps—helping reduce stress while ensuring care aligns with Indiana Medicaid requirements.
